How do i open these? whenever i click on them they always just open up internet explorer and download itself again

If it still in rar format you can uncompress it using winrar or 7zip.
For the gba file you open itu using emulator like Visual Boy Advance or maybe retroarch if you want to play with shader (if you just want to play without hasle I recommend you use VBA).

As far as I know it only add girls sprite as pokemon sprite (reguler pokemon still exist, it's not like moemon where all pokemon sprite is changed to moe chara sprite).

Regarding full on sex scene I don't think there exist, it some times show pop up of pokemon sprite (like the one that show for player first pokemon). Maybe because it limited to what GBA can do?
