You pretty much nailed it in the end. The issue is "corporate" for lack of better term. Japanese creatives are really strict about chasing specific target audiences and rarely stray from that. Which results in pretty much putting out the same product (with a different coat of paint) again and again, locked in their genre box. Same plot, same tropes, same scenes, profably different color hair for the fmc. It's the same for their mangas, their anime and ofc their games. Lately I'm starting to see the same with western adult games too.

Well - For people that cannot read any Japanese, it's at least something.

It makes the game from fully unplayable (because you cannot understand a word of it), to at least playable. And to be honest, I rather have a slightly improved MTL ("sugarcoating" as you called it) than a "raw" MTL. But if that "sugarcoating" is not available, I rather have a "raw" MTL than nothing at all.

Swapping is a very hard tag to do right, it usually always goes to NTR or cuck eventually. From my experiance the Korean pornmangas are the ones that do it best, though they love their NTR there too and usually takes it that route as well, but I'm always on the hunt for a good netorase game where the girl loves what hse does but also never stops loving the guy.
