My 2 cents ...
I don't mind the harem escapeism but it seems like you have a new writer. Waaaay to many repeat conversations and they were superfluous at best. Way to much tell and not show. It seemed like everybody had to do a monologue of what their morals and actions were going to be. Sometimes this happened 2 or 3 times with each person. You have mostly developed these people so we don't really need this. Its like all the badasses are quoting Shakespeare to justify their reasoning and it is not needed. The earlier story was just great. Still in there with you though but man....... I hope we get back to normal soon

Really, because then just about every VN on this site would disagree with you. I hate to break it you but having plots porn movies would laugh at as AVN premises do not exactly scream 'realism'. Most tags on every VN alone would disqualify them as being realistic.
That said, it would be nice to have plots that inherently make sense, dialogue and grammar that are easy to understand and characters we can get invested in.
That may be true but it doesn't mean we should fantasize illegal and immoral things like rape and corrupting young women to becoming sluts and whores, life should be better than that

The story is somewhat ok, the renders need work, some characters are just left out and the worst is the writing. The conversations are unnecessarily long, repetitive and boring.
The conversation with Reina was particularly boring, it looks like the only thing they are talking about is how Yakuza's tradition and culture is and how MC is reassuring that he would do his best in everything and blah blah blah and it keeps on rerepeating over and over.
The next one with the cops was plain stupid, I mean they are investigating a crime scene where a corpse is lying on a bed for a few days giving foul odor and they are just casually chitchatting like they are in a café for a get-together.
MC is portrayed as the strongest and a complete badass but he is written very poorly. He is way too nice to everyone and a Grade-A pussy, what he needs is a bit of aggression cuz in a world he's living in, others would perceive him as weak and pathetic. Even a simple grunt is threatening MC that if he doesn't treat his lady good they'll have problems. I mean he called MC there, requesting him not to kill his sister and there he's also threatening him. Seriously, WTF.
Honestly, there are lots of areas for improvement but the main thing to improve is writing which is total shit right now.

Dunno why this has an incest tag. A peep on sis while she squirts while jilling off and just thinking about her bro, a kiss for a cousin, a kiss for a drunk mom or just rubbing grandma's feet shouldn't count (sis should have fucked him or at least a handjob after the kidnap rescue). 26 months later and that's still all there is. By now I'd expect at least a few fucks if not a creampie or 2 and everyone getting knocked up including grandma and every other family member My Dorm style with the MC's super-swimmers (powerful enough to win the fight against birth control and even swim through condoms I'd assume). Along with renders that look 5yo (at least the animations are not 2-render flipflops, but only slightly better and some are still quite grainy) no wonder this only has 2.5 stars and I dunno how this has 170 Patreon subs, must be all $1 ones they forgot about.

Sadly entire (stupid) countries like Australia/Canada/UK disagree with you. Ie loli AVNs. Cops don't like it they will not only play it anyway but hunt down and arrest the dev for it with that exact thought that it shouldn't exist.
I ended up just ctrl skipping it after prob 6000 or so dialog lines in and even the sex scenes are "meh, might as well skip those too if Emma or Mom isn't in them" which of course means all of them since they don't have any sex scenes, just a couple of nude ones/peeping. Another case where the incest tag is mostly bullshit clickbait.
An AVN based on a true story would definitely be something new, as "this AVN is ficticious and any similarities with real events are purely coincidental" is way past getting old as fuck, someone should make the opposite for a change and based entirely on a real person and real events, legal problems of that be damned (AVNs are niche things and not Lifetime TV movies, odds are no one important would notice anyway and just take crypto or other anonymous payment on Backed.by if need be). Want alot of sex in it just make it a biography and MC is a famous porn star like Randy West, Ron Jeremy, etc. and include the shit they are doing when not on set. For a darker theme just make the MC a serial rapist and/or total psychopath based on Ted Bundy, Ed Gein, John Gacy (if going for the gay tag) or similar. Or base it on the exploits of a long list of busted real child rapists/molestors if going for loli/shota tags. A sex offender/criminal records search alone will turn up more material than can possibly be used in even 2k AVNs. As they say sometimes real life is stranger/darker than fiction.
What i find funny is the beginning they are so important but he barely talks to them at this point this game lost the point a long time ago. The mc has more interaction with random one off characters than the family he was separated from and supposedly loves so much. I gave up on the family when the mc got hurt and the mom showed up for 5 sec and told to go home and that was it. the mc almost died and that was it showed up told to go home and then ignored. This game started fine but is so bloated and just goes from one thing to the next. And with the game jumping so much and going from random plot to the next the family has no skills to aid in whatever new drama happens so they are pointless. If they were removed it would hardly affect the game at all.
Yeah exactly. Like the pool scene Emma's having an inner monolgue soon after getting rescued and before that MC peeps on her while she's jilling off/squirting and then nothing as far as escalating the sexual tension or whatever, for update after update nothing with her until she simply mentions a dinner date with MC and their mom. I guess she wasn't in love with her bro as much as she thought or something and got over it. Or at least that what it comes off as. It's like an orchestra build up to silence. Hell, worse than that, like Tchaikovsky's 1812 overture, but with the cannons replaced with wet-fart sounds made with armpits. Oh and 2 conductors instead of one. Even with no sex at all fans of vanilla James-Bond-style spy or Mafia stories will probably be disappointed. As far as Mafia theme even The Guardian was a bit more consistent, especially after the 0.12 rework. But yeah this one has gone off the rails even more than Thristy For My Guest (aka TFM Daddy) that I cut my losses on after an LI inexplicably simply dumped the MC and left town not long after getting engaged to him (or something like that, been a few years).
Like many games before it, this game's #1 issue is simply adding too many LI's too quickly and giving undue focus to too many of them, whilst neglecting half of the original cast.
For what it's worth, Thirsty For My Guest is pretty good, especially the H-scenes, although it is quite silly. The dev said the girl that got removed was for personal reasons, so make of that what you will. I would recommend playing it just for Jenna and Vanessa alone.
