Preg is by event only. Two scenes for this, birth included. One bug, one tentacle monster+egg.
There's a fair bit that isn't translated at all as well, mostly in the recollection room.
This isn't a big game either, kinda grindy. Can finish in litterally less than 5 mins. It doesn't stop you from immediately stomping the boss and getting the good ending. Porn endings take a a bit of grinding, can probably get one of those in about an hour or so.

The next time you fight the boss he'll have a bit of extra dialogue and will give you a new outfit, which if you wear it is reflected in quite a few scenes.
More on topic, I enjoyed the game and the MTL is perfectly fine for getting through it, it's not gonna win any awards, but I understood what was happening.
I actually really like the option to show the unlock conditions in the gallery rather than just unlocking everything, for a game like this I find the most fun in trying to hunt down all the scenes rather than just unlocking them all.

Don't bother with the game. The game play is you just running around trying to find events, finding out that you don't have the required "Stats" or previous required item, scene, etc unlocked. To raise stats you need to do scenes. There are about 7 endings and the variation on CG's for a set is almost non-existent for the majority of them. The endings let you restart from the point before and farm for more points to put into the stats for scenes. They also are mostly around 3-5 cg's. I wish I was exaggerating when I said there's barely any real penetration in this game...
To each their own, but hours in this game for what we got isn't worth it. Just unlock the gallery and get it over with...
