In answer to my own question about the twins, its random, when you leave your house one of the twins comes up to you and tells you she preggers, you can choose to abort or not, if you choose not to im assuming when she is far enough along you get the preggers scenes

You're using the player to run the qsp file, which you shouldn't for this game. Use the "Son of a Bitch.exe" in the main folder.
