it's nothing more than punishing the player because he played as intended (he made the MC have a lot of sex).
that's actually exactly what a storyteller should be doing, stopping mc from getting his way. that's what creates the 'engaging characters' people always claim they want. where as mc getting his way produces those flat characters and superficial stories people claim they hate.

but yeah, I can't really see it working with std. even in the brothel scenario pregnancy can do all the same things mechanically, with far greater emotional potential for both good and evil.

It's not that you can't see it working with STD, it's that STD are something radically different.

What you talk about is girls that are reluctant, and the MC being stopped, by someone entering the room, a phone call, or whatever, when he finally achieve to take a step further. This until the moment he will finally achieve to conquer the girl.
But STD aren't at all that. You talk about everything that happen before the player get his reward, while STD are happening after he get it. "Congratulation player, MC have got one more pussy. Achievement unlocked, you get a STD..."

There's not many interesting gameplay revolving around STDs. If you want to make such mechanics, there are other alternative no-nos better than STDs like pregnancy. Porn Empire has STDs but it does not go further than medical check for your pornstars.
