The game is mostly meant to be played with your mouse which means that if your mouse is anywhere near an option it'll usually get stuck there.
beyond that its Z to confirm and X to cancel. I think X also opens the menu if you're in town or in the dungeon. If your mouse is thoroughly off the screen you can select things using the up and down arrows instead of clicking on them
