What do you mean transformation?
If you're talking about bellies, yeah they do have pregnant bellies in the epilogue of the game.

Normally its means m to f, f to m or male or female to futa. Some kind of gross change to the body or changed into a monster or things like this.
Showing pregnant bellies is not what the tag means.
Another game to watch for: Perfect Family
(1) Ren'Py - Perfect Family [Update3] [POHG] | F95zone
It's still early on, so there's no pregnancy content yet, but it's in the Future Tags (also not a lot of content yet in general). And you have to activate incest mode by choosing to alter relationships and declaring the first woman to be 'Mom'. But once you have, the main succubus character casually mentions she's the product of her mom and her mom's sister after drinking a futa potion. And they look like they could be twins.

Not sure if it's worth mentioning, but for Peasant's Quest there is B/S but they're half-siblings (same biological father). Iirc Luke's Way also has the choice for M/S (with patch), though nothing visible yet but the in-game hint system does confirm the path is there. Should be noted that was implemented I think since early 2021 so anyone interested should tame their expectations in terms of progress. Motherless I believe is missing the cousins tag but considering how screwed up the family tree in that game is it would probably just be easier to make a new category for it like "incest incarnate".

Yes it needs to be added to Peasant's Quest that it is also B/S.
As for Luke's Way it needs to be confirmed in the game, just waiting on it to add to the list, I also hope we will get to inpregnant the sister to.

Luke's way has a pregnancy route for the mom and will most likely have one for the sister in the future too

Its hard to put a rating system for this because there is to many different styles here.

I agree that the above kinks should be listed so players who want to ignore that type of kinks can do so.
