You get them from Doctor if you fill some Vials with semen and also if you have enough Favor of Doctor... Keep on visiting Doc often and keep on Wanking in your room

For every 5 high quality semen samples you deliver, you get 1 blue pill.
For every 5 low quality semen samples you deliver, you get 1 green pill
High quality semen is when you jerk off with lust higher then 50.
Low quality semen is when you jerk off with lust lower then 50.

I believe that you need to fill three vials full of semen while at high lust. If you do it while your lust is at last 70 percent it should work.
The status window on the left hand side of the window shows your lust status. You'll be able to see if you'll produce an EH, an H, or a L vial of sperm from that.
I checked the code. In order to get a blue pill, you need to deliver 5 H vials. H is high lust but not while under the effects of a blue pill.

And to complete it.
You produce an E vial when you are under the effect of the blue pill but under 50 lust.
