All the things you mentioned can and are ways people fuck. Even if you look at the less extreme of it there are people that engage in rape roleplay.
The issue here is, where does the attraction to literally shit yourself come from?

I'd say that's a pretty narrow view of what a fetish is. People find a lot of things arousing/sexual that have nothing directly to do with fucking. Also as far as my understanding goes not all ADBL people are into shit stuff, it's more about the pretending to be a baby thing for some people. Why do they find that hot? I don't know. But some people want to dress up in weird latex outfits while they fuck and I don't get that either. People are different.

It seems to me the tag "fetish" may be a tad bit generic.
