I got the same problem, if you are using an old file you screw up. You get the permit to go to the island when you rescue the elf girl and than talk when she is on the bar. If she is upstairs you self lock.

All i can tell about the 0.2.1 update is that it just adds the pregnancy function, dont know about any new missions/bosses etc. outside of some impreg scenes and stuff
