There was that one incest game where you travel back in time, fuck your grandma and basically become your mothers father. I think you fuck your mom later on too and she doesn't know you're her father. Technically that fits the bill.

Other than that I'm not sure.

Hah I was gonna suggest this anyways for the fact that the first two times you fuck your mom you also turn into your father. So yeah, transformation, m/s, and f/d all of which unknown to her at different times.
